The Four A's receives funding from multiple sources, each with their own eligibility criteria. The core eligibility criteria for any service through Four A's Client Resources department is an HIV positive certification.
All individuals requesting services from the Four A's are required to complete an intake process with Four A's staff prior to any service being provided. Intakes can occur over the phone if an individual is able to demonstrate a hardship would be caused to meet in person (i.e. residence in village, or cost of travel to the Four A's office in Anchorage).
At your intake meeting be prepared to provide a copy of your HIV certification from your medical provider. You are also expected to provide income documentation of one of the following:
At least 2 months of consecutive paystubs
Most recent tax return
Declaration of No Income
Your intake will include a basic demographic and HIV specific interview, development of a service plan, and completion of any releases of information required to advance the supports requested. Be sure to allow ample time for the intake meeting as it takes time to complete all paperwork and initial service planning.
